Independent Contractor - Version 2 (CO Attorney Reviewed)

A nurse practitioner specific independent contractor contract, created by a Colorado attorney at the cost of $1200.  This contract can be used for content, or perhaps as a stand alone document.  Modifications to the document will need to be made after download to work for your practice.   This is not a "fill in the blank" type of form.  You will need to use a PDF Editor to make necessary modifications.
